REGISTRATION This form is to be used for Faith Formation Programs: weekdays and Summer Plus program. The Forms are available in the Faith formation Office. New students and transfer students must also complete a NEW Family Information Form.

download and read the Faith Formation Guidelines for our classes. Also included are Summer Session Guidelines and Volunteer Forms. Please consider helping out either in the classroom, as a hall monitor, or as a snack aide.
Faith Formation Guidelines Summer Program There will be a parent meeting on Wednesday, May 22nd, at 6:30 PM in the school auditorium, for those parents interested in having their children attend the Faith Formation Summer Program. At this meeting the guidelines will be discussed and any questions will be answered. The summer program is available for grades 1 to 6 only. Registration forms will be available only for those parents attending this meeting. The registration form, together with the registration fee, can be returned to the Faith Formation Office on Friday, May 24th, from 9 AM to 12 Noon.
Please remember there is a limit of 20 children to a class and if the class is filled, it will be closed.
Please note: No registration forms will be accepted prior to this meeting date either
in person, by mail or by fax.
SUNDAY PROGRAM:
Class Timings: Sundays 8:30 – 9:45 AM followed by 10:00 AM MASS
There will be parent meetings as follows for those parents interested in having their children attend the Faith Formation Sunday Program:
Grades 1 to 4 - Wednesday, June 5th, 6:30 PM - school auditorium
Grades 5 to 8 - Thursday, June 6th, 6:30 PM - school auditorium
Registration forms will be available to those parents attending the meeting and can be returned with the registration fee to the Faith Formation office either in person or by mail prior to August 1, 2013.
